When a root cause analysis (RCA) is conducted utilizing the five WHY approach, each “Why” should be accurately recognized and analyzed because putting root cause analysis into practice and reaching a consensus are challenging undertakings.
In many manufacturing, engineering, and non-engineering firms, root cause analysis (RCA) is currently an extensively used and accepted tool. When problems have several open endings, a strategy like 5Why is more applicable and has a significant role to play because of its simplicity and convenience of usage.
