The journey of a product or service begins at the conception stage, when the objectives are clearly identified and specifications determined. Through the various stages of the product life cycle, though, many changes may happen due to various factors such as availability of raw materials, the manufacturing capabilities, customer feedback and so on. As multiple functions are involved in the creation of the product/service, there can be times when there may be a deviation from the original specifications, leading to nonconformance. This must be documented and investigated to identify the root cause of the variation, and corrective action taken. This is called a nonconformance report, and it helps with improving the product quality as well as compliance with internal and external standards. This is especially important for Life Science industries, such as pharmaceuticals, medical devices, and laboratories due to the sensitive nature of their products and the industry being highly regulated.
The nonconformance report should be a comprehensive document that meets regulatory requirements and should indicate whether it is a minor or major nonconformance. Some of the key elements of the report include:

Step 1: Determining Nonconformance Identifying products or processes that do not meet the standards, requirements, or specifications is the first step of the reporting process. This could be determined based on rejection of a batch at the testing stage, customer complaint, audit, or observation.
Step 2: Assessing the Nonconformance The nonconformance may be major or minor, and this will decide what should be done next. Minor nonconformances do not impact the safety, quality, or performance of the product and may be acceptable to regulatory authorities and customers. Major ones cannot be ignored due to safety and performance implications.
Step 3: Create a Nonconformance Report The next step is to document the deviation with all the essential details mentioned previously.
Step 4: Notify Authorities Notify the relevant authority, such as senior management or a review team, to inform them of the issue and decide on the next course of action.
Step 5: Perform a Root Cause Analysis Investigate the nonconformance to identify the root cause before deciding on the next action. Sometimes, it may seem that the reason for the nonconformance is one thing, but investigation may reveal the root elsewhere. For instance, a process may have caused the deviation, but the investigation may reveal that the equipment was not calibrated or the raw material was not as per specifications. Knowing the root cause is essential for effective correction and prevention.
Step 6: Taking Corrective and Preventive Actions Identifying the root cause helps decide which is the next best step to correct the situation and prevent future recurrence. This could involve calibrating the equipment, changing suppliers, providing training, and so on.
Step 7: Monitoring for Success Is the corrective action performing as expected and delivering the desired outcome? This must be tracked and monitored to ensure that the CAPA is successful, that the nonconformance does not recur, and that a new issue has not been identified because of the ch
