Why Traditional Safety Metrics Still Miss Serious Risks

A few years ago, in our whitepaper Understanding Leading and Lagging Indicators for EHS Excellence, we argued that safety departments need to look beyond traditional safety metrics and build a stronger mix of leading and lagging metrics.

As EHS programs mature, we believe another question is becoming increasingly important:

Are we measuring the risks most likely to seriously hurt people? Can we proactively prevent major mishaps?

The truth here is that despite decades of investment in safety management systems; serious workplace incidents continue to occur, particularly in high-risk industries such as manufacturing, aerospace, chemicals, and life sciences.

Workplace safety has improved in many measurable ways, but the most severe outcomes remain stubbornly difficult to eliminate. In the United States, more than 5,000 people still die from work-related incidents each year, even as broader injury rates have trended downward.

That disconnect is one of the issues the National Safety Council is addressing through its Work to Zero initiative, which explores how research, technology, and evidence-based safety practices can help organizations identify and reduce the risks most likely to result in serious injuries or fatalities.

A recent chemical safety crisis at an aerospace plant starkly illustrates the gap between managing routine safety events and addressing high-risk incidents with potentially catastrophic consequences. In May 2026, temperatures began rising inside a tank at an aerospace company’s facility in California, after a valve in its cooling system failed. The tank contained roughly 7,000 gallons of methyl methacrylate; a volatile chemical used in manufacturing plexiglass. Authorities warned of the potential for a massive explosion, and evacuation orders affected more than 50,000 nearby residents before the situation was brought under control.

What ultimately became a near miss had the ingredients for a dramatically different outcome.

The point is this: a workplace can have an excellent TRIR, declining lost-time injuries, strong training completion, and a healthy volume of reported near misses and still carry significant exposure to a low-frequency, high-consequence event.

The next evolution in safety measurement, then, is not about abandoning traditional metrics. It is about adding metrics that answer a different question: Where do we have the risk for something serious to happen?

Traditional Safety Metrics vs. the Metrics That Matter for Serious Risk

Traditional metrics remain valuable. They help organizations understand overall safety performance, compliance, reporting discipline, and injury trends.

But when the objective is preventing serious injuries and fatalities, EHS leaders need another layer of measurement, one focused on exposure, severity potential, control effectiveness, and emerging risk.

You may have already noted the following from the table above:

  • Traditional metrics tend to measure outcomes and activity.
  • Risk-based metrics increasingly measure potential and exposure.

Key Point of Note: Not All Safety Events Are Equal

Consider two near misses.

  • In one, an employee trips over an electrical cable but catches themselves before falling.
  • In another, a pressure-control system fails on equipment containing hazardous chemicals, but a secondary protection system prevents a release.

Both may technically be recorded as near misses. Operationally, however, they are not remotely equivalent. One suggests an everyday workplace hazard. The other may expose a weakness in a system designed to prevent a fatality, explosion, toxic release, or other catastrophic event.

Counting both events as "two near misses" tells management very little about the organization's true risk profile.

What matters is not simply whether an event occurred, but:

  • What was the potential severity?
  • What hazardous energy was involved?
  • Which critical controls failed or nearly failed?
  • What prevented the outcome from becoming worse?
  • Could the same condition exist elsewhere?

Leading Indicators Need to Become More Intelligent

The safety profession has already made an important shift from purely lagging indicators toward leading indicators.

But there is another evolution underway. Simply counting more proactive activities is not enough.

Consider the difference between these two metrics:

Vs.

Critical-control inspections identified repeated degradation of three safeguards associated with high-energy equipment across five facilities.

Both are leading indicators. Only one provides management with meaningful insight into serious risk. The same applies to observations, training, audits, corrective actions, and near misses.

If it’s not obvious already, safety management is becoming more and more about operationizing the aspect of risk identification and risk management.

From Safety Metrics to Safety Intelligence

Traditional EHS environments have largely been designed to collect data, report metrics, track incidents, monitor compliance, and review the past.

The opportunity now is to build more intelligent EHS environments that can connect data, explain patterns, identify SIF potential, prioritize high-risk exposures, and guide decisions before something serious happens.

This is ultimately the shift that matters: from measuring safety performance primarily through what has already happened to building the intelligence needed to understand what could happen next and where intervention matters most.
